New Patients
Thank you for choosing Gyn Care for your health care needs. In order for us to provide you with the best possible service, please be sure to pay special attention to the items and instructions listed below. - Please complete the Registration Form and email it back to us at info@gyncareclinic.com. Please type “New Patient Registration” in the Subject field so that we can easily identify your email. We will digitally collect your signature during your initial visit.
- Review the following documents, and we will digitally collect your signature for each document during your initial visit:
- If you would like to send us a copy of your medical records from another health care provider, please fill out the Medical Records Release form and email it back to us at info@gyncareclinic.com. If you have not included it with the "New Patient Registration" email, then please type “Medical Records Release” in the Subject field so that we can easily identify your email. We will digitally collect your signature during your visit.
- Bring your current active insurance card and driver’s license or other state or federal authorized picture ID card. If we cannot validate your insurance coverage and your identification, we will have to reschedule your appointment at a time when you can provide us with valid insurance and ID cards.
- Bring your co-pay amount (cash, *credit or debit card, or check) if applicable.
- If you do not have insurance or are seeking care outside of your insurance plan benefits, payment IN FULL is requested at the time of service. Our staff will gladly give you an estimate of your visit prior to your appointment. If you are interested in making arrangements to have the balance paid in full by your appointment date, please contact our Billing Supervisor at (337) 480-0140 or toll free at (800) 689-2619, ext. 5. For your convenience we accept cash, checks, money order, and *credit or debit cards.
- Please arrive 20-30 minutes before your scheduled appointment time so that we can process your forms, co-pay and validate your insurance so that you can be seen in a timely manner.
- If you arrive late for your appointment we will try to accommodate you but you will be delayed. If you arrive 30 minutes or more late, we will have to reschedule your appointment.
*We accept all major credit cards—VISA, MasterCard, Discover, and American Express.
